Laparoscopic approach for the management of bilateral post-TAPP mesh infection in a patient without risk factors case report

Abstract
Although prosthetic infection following a transabdominal preperitoneal approach (TAPP) is uncommon, it presents a significant therapeutic challenge. We present the case of a 47-year-old immunocompetent male patient with no comorbidities who developed bilateral deep mesh infection (DMI) after laparoscopic inguinal hernioplasty. After the failure of conservative management with antibiotics and percutaneous drainage, laparoscopic explantation of both prostheses was performed, with a favorable postoperative course and no recurrence during follow-up. This case illustrates the relevance of bacterial biofilm as a mechanism for the failure of conservative treatment and the role of minimally invasive surgery as a definitive therapeutic option.
